Effective home management of chronic conditions—from dementia to heart failure, metabolic disorders to circulatory and pulmonary conditions, and even incontinence—is essential to make this possible for patients and caregivers alike.

In this challenge, consider how your digital health or connected device solution can benefit all stakeholders—patients, professional and nonprofessional caregivers, clinicians and payers—across the patient care ecosystem.

About the Challenge

Identify new concepts in Digital Health that can prevent health crises and better managing chronic conditions. More broadly, the Connected Patient Challenge is designed to foster a digital health innovation community where discussion with input from many sources can build strong, robust challenge submissions leading to successful, life-changing diagnostics, devices, therapies and patient management programs.

We aim to:

  1. Bring together people (healthcare provider individuals and institutions, patients, payers, researchers and industry) from across the healthcare ecosystem to learn pain points within healthcare and to discuss digital health solutions.
  2. Explore the potential of using patient or population health data, patient monitoring and/or predictive analytics to ultimately improve patient care.
  3. Consider how these solutions can support a value-based care paradigm to improve patient outcomes, make care more efficient and ultimately lower healthcare costs.

Types of submissions we are looking for


Practicality

Does the presenter clearly state the problem they are solving? How does their idea solve the problem and how will they evaluate how well their solution works? Can the idea be realistically implemented in today’s clinical environment at a reasonable cost?

Efficacy

We are looking for ideas that produce real benefits (improved patient outcomes, cost saving, reduction in care needed by patient, better patient experience, etc.).

Areas of focus

Concepts that are focused on the following disease states are most attractive: cardiovascular; digestive; cancer; neurological / chronic pain; respiratory; and/or urology and pelvic health.
